• Letter of Eligibility (physical or email) or online portal confirmation.
• Electronic Benefit Transfer card (EBT), front and back.

Document requirements

• Must include the first and last name of the parent or guardian applying.
• If the document doesn't include the parent or guardian's name, their ID will be required.
• Must be issued within the past year or not be expired.
• Must include the name of the program.
